MED3OOO partners Phytel to develop proactive patient care software

MED3OOO and Phytel have agreed a partnership aimed at providing physicians with the tools needed to proactively improve patient care.
Both companies will expand their offerings and embed additional features into the proprietary product line, M3/Connect powered by Phytel.

'The affiliation with Phytel is key to our long-term strategy of providing information and technology that proactively improves patient care while enhancing our clients' growth strategies,' said Jay Anders, MD, MED3OOO chief medical information fficer. 'The alliance will give us the opportunity to work together to produce future tools to help our physicians better serve their patient populations.'

The M3/Connect powered by Phytel is a patient registry based solution that improves coordination of care by automating the follow up process for a practice's patient population, specifically patients with chronic conditions and those in need of preventive care services.

M3/Connect powered by Phytel aggregates information from MED3OOO's M3/IQ warehouse, Practice management systems, electronic medical records, hospital information systems and other available data sources in order to create an active patient registry for the practice or organization.

M3/Connect powered by Phytel applies evidence-based disease management and preventive care protocols against the registry data to determine which patients are due for care. Identified patients are notified through automated outreach messaging. M3/Connect powered by Phytel also enables practices to track patients' responses and monitor compliance.
