Orion Health customers receive recognition for innovations in Chronic Disease Management

Calgary Health Region and Capital Health of Alberta, Canada, and Procare of Auckland, New Zealand have both received awards for the Best IT Projects in Chronic Disease Management at the Calgary International Chronic Disease Management Conference.
“We're impressed at just how much value the technology approach to chronic disease management is delivering for a number of clients, and we're pleased they have been recognised for their diligence and continued hard work in this area,” said Dr Chris Hobson, Orion Health chief medical officer.

“Technology's role in CDM is finally coming of age and being recognised as a key driver, not only in the traditional areas of cost improvement, but as a recognised part of clinical practice and patient care. Orion Health clients are very much in the forefront for implementing disease management internationally, pulling together common threads and demonstrating the true scale of what can be done in this arena.”

Calgary Health Region and Capital Health of Alberta jointly won first prize for the Best IT Project in Chronic Disease Management in Canada . The regional chronic disease management project, initiated in 2002, features an innovative digital dashboard project unifying disease management across the entire province. With more than 40,000 patients under care and including home care and family physician involvement, the system assists patients and physicians with the management of diabetes, hypertension, COPD, pain, anticoagulation management and community care.

Procare, Auckland , New Zealand , received the international award for the best Chronic Disease Management Program the conference's highest accolade in this category. In 1997 Procare recognised a rapid and unsustainable growth in admissions and ambulatory response. By implementing a chronic disease management programme the organization was able to dramatically reduce the growth in acute medical admissions from nine percent to zero percent.

Also presenting at the conference was Orion Health customer Alere Medical, who demonstrated the company’s approach to using technology to improve care and reduce costs for patients with chronic diseases including heart disease, Coronary Artery Disease (CAD), Chronic Obstructive Pulmonary Disease (COPD), asthma and diabetes.

“Alere gave a great presentation and clearly impressed the audience with their ability to manage data sent from home based medical devices such as scales, glucometers and blood pressure monitors,” said Dr Hobson. “Alere's innovative approach demonstrates how significant technology can be in managing costs and improving patient outcomes.”
